About the Role
We’re looking for a compassionate problem-solver who thrives in a mission-driven, team-oriented environment.
The Case Manager serves as the central hub of client care at The Ark. This role helps individuals and families navigate complex challenges with empathy and skill—developing collaborative care plans, coordinating internal and external supports, and helping clients move toward self-reliance and stability.

What You’ll Do
• Serve as the “hub” of client support, coordinating care across social services, medical, housing, and financial programs.
• Partner with clients to create individualized care plans aligned with their needs, strengths, and goals.
• Provide motivational interviewing, active listening, and basic crisis intervention.
• Assist clients with public benefits applications (SNAP, Medicaid, LIHEAP, SSI, etc.).
• Maintain accurate, timely documentation and ensure continuity of care.
• Participate in team meetings, trainings, and ongoing professional development.

What You’ll Bring
• Education: Bachelor’s degree in Social Work (BSW) or related human services field required; MSW preferred.
• Experience: 2+ years in case management or social services.

Skills:
o Strong communication, rapport-building, and organizational abilities.
o Familiarity with motivational interviewing, trauma-informed care, or crisis intervention is a plus
o Knowledge of community resources and referral systems.
o Understanding of basic mental health symptoms for screening and referral.
o Cultural humility and appreciation for Jewish values and traditions.
o Proficiency in Microsoft Word and Teams.
